- Pay Starting at $24.00 - $26.00 an hour

- Engage clients in developing daily living, recreational, and independent living skills
- Monitor clients’ needs through observation, check-ins, and ongoing communication
- Implement individualized treatment/service plans (ITP/ISP) and report changes or concerns
- Support symptom management, coping skills development, and community integration
- Assist clients in accessing community resources, healthcare providers, and benefits
- Provide advocacy, outreach, and re-engagement support for missed appointments or disengagement
- Respond to and help stabilize crisis situations using appropriate interventions
- Maintain accurate, timely documentation in the electronic health record, including assessments, progress notes, and treatment plan updates
- Participate in team meetings, supervision, training, and collaborate with clinical and support staff to ensure coordinated care
- Minimum of a bachelor’s degree in social work, Counseling, Psychology, or a related field
- Must meet Ohio MHAS criteria for Qualified Mental Health Specialist (QMHS) if not licensed
- Valid driver’s license and reliable transportation required
- Must be insurable under the agency’s commercial liability policy
- Experience working with children and families preferred
