Admissions Hospice RN

In this field-based role, you’ll assess patient eligibility, perform comprehensive nursing assessments, educate patients and families on hospice philosophy and services, and develop initial care plans in collaboration with the interdisciplinary team. You’ll communicate with referring providers, ensure timely and accurate documentation, and support smooth transitions of care.
