Job Description The Case Manager, CARE Team reports to the Manager of the CARE Team and is responsible for assessing student needs, triaging complex situations, and coordinating timely referrals to campus and community resources. This role serves as a primary connector between students and the support services necessary to address wellbeing, behavioral, safety, academic, and basic needs concerns. The Case Manager provides direct case management services that promote student success, persistence, and retention through coordinated intervention and support efforts. The position requires a strong understanding of student support services, behavioral intervention principles, campus resources, and community partnerships. Working collaboratively with students, faculty, staff, campus leaders, and external organizations, the Case Manager assists in identifying barriers to success, facilitating access to appropriate resources, and supporting students through complex and sensitive situations. The role contributes to the mission of the College through professionalism, sound judgment, collaboration, and consistent delivery of quality services and outcomes. The Target Hiring Range for the position is $ 57,542 - $62,000 The final salary offer will be determined based on the candidate's qualifications and experience. Essential Functions The employee will demonstrate TSTC’s Core Values of Excellence, Accountability, Service, and Integrity with internal and external stakeholders, customers, students, and community members. Case Management & Student Support Conducts comprehensive intake and needs assessments to evaluate student concerns, including behavioral, wellbeing, safety, academic, and basic needs issues. Coordinates referrals and connections to campus departments, community organizations, and specialized support services based on individual student needs. Maintains ongoing outreach and engagement with students to assess progress, provide support, and encourage follow-through with recommended resources. Coordinates, tracks, and monitors students identified as being at risk of discontinuing enrollment and works collaboratively with campus and statewide partners to remove barriers and improve student outcomes. Supports students across assigned campuses while ensuring equitable access to services and resources. Maintains current knowledge of student support services, emerging trends, available resources, and best practices that support student success and persistence. Collaboration & Communication Builds and maintains collaborative relationships with campus departments, faculty, staff, statewide partners, community organizations, and external service providers to maximize resources available to students. Communicates objectives, expectations, timelines, and recommendations clearly while fostering productive partnerships focused on student success. Responds promptly to student inquiries received through various communication methods, including email, text, telephone, virtual platforms, and in-person interactions. Provides presentations, training, and informational sessions for students, faculty, staff, and community stakeholders as appropriate. Participates in CARE Team meetings, committees, advisory groups, and collaborative initiatives that support retention and student wellbeing efforts. Case Coordination & Risk Assessment Maintains accurate, timely, and detailed case records and documentation for assigned cases and related communications. Conducts risk assessments and assists in the development of safety plans for students of concern. Coordinates welfare checks, emergency responses, and other intervention activities when warranted. Participates in case review meetings and provides updates, recommendations, and status reports regarding student concerns and intervention strategies. Exercises professional judgment and discretion when managing sensitive student situations and communicating with stakeholders. Quality & Compliance Ensures work products, case documentation, referrals, and services comply with institutional policies, applicable regulations, confidentiality requirements, and established case management standards. Maintains appropriate records and documentation practices that support accountability, consistency, and effective service delivery. Continuous Improvement Identifies opportunities to improve case management processes, workflows, student support strategies, and service outcomes. Participates in professional development opportunities related to case management, behavioral intervention, threat assessment, student wellbeing, and higher education student services. Applies new knowledge and best practices to enhance effectiveness and improve student outcomes. Education & Experience Minimum Qualifications Bachelor’s degree required. Two years of experience working in student services, case management, behavioral intervention, retention services, counseling support, social services, higher education, or a related field. Experience supporting students through complex situations requiring assessment, referral, coordination, and follow-up activities. Preferred Qualifications Master’s degree in Student Affairs, Higher Education Administration, Counseling, Social Work, Psychology, Social Sciences, or a related field. Experience supporting behavioral intervention, CARE Teams, student conduct, threat assessment, or case management functions within higher education. Relevant experience may be considered in lieu of formal education where appropriate, consistent with TSTC’s equivalency philosophy. Skills & Abilities Ability to assess student needs, identify barriers to success, and coordinate appropriate interventions and referrals. Strong analytical, organizational, and case management skills with the ability to gather, evaluate, and document information accurately and confidentially. Excellent written, verbal, and interpersonal communication skills with the ability to work effectively with students, faculty, staff, community organizations, and external partners. Ability to exercise sound judgment, discretion, and professionalism when managing sensitive or high-impact situations. Strong problem-solving, decision-making, and conflict-resolution skills. Ability to maintain detailed records, prioritize multiple responsibilities, and perform effectively in an environment with frequent interruptions and changing priorities. Knowledge of student support services, higher education operations, student wellbeing resources, and behavioral intervention principles. Ability to contribute to threat assessment processes and support the implementation of behavioral intervention strategies that promote student safety and success.
