PT Role Description:Our new PT will be responsible for the evaluation and treatment of a variety of diagnoses and related impairments.Evaluating a patient's functional statusEstablishing treatment goalsPlanning treatmentImplementing the plan of care through direct treatmentProviding instruction and education to patient, family, and caregiversEvaluating need for interdisciplinary referralParticipating in multidisciplinary patient care conferencesProviding accurate, timely documentation of treatment
Qualifications:Masters Degree in Physical TherapyIL State License1-3 years of pediatric experience preferredComfort communicating with medical professionalsEnergetic, compassionate, positive attitude with patients and familiesEnjoys working in a team environment with PT/OT/ST and support staffExperience with EMR a plus
