Community Care Physicians seeks an Outpatient Nurse Practitioner or Physician Assistant to deliver patient-centered care in our multispecialty, physician-owned group in Latham, NY. In this role, you will manage acute and chronic conditions, perform exams, order and interpret tests, prescribe medications, and provide preventive and wellness-focused care. You’ll work closely with physicians and an interdisciplinary team, supported by robust clinical resources and an EHR, with opportunities to grow across multiple specialties and outpatient centers in New York’s Capital Region.
