TLC Nursing Associates, Inc. is seeking an experienced Case Management Registered Nurse (RN) for travel assignments. This role is ideal for RNs who specialize in coordinating patient care, ensuring continuity of care across healthcare settings, and optimizing patient outcomes. Responsibilities

  • Assess patients' medical needs and develop individualized care plans
  • Coordinate patient care across different healthcare providers and settings
  • Collaborate with physicians, therapists, and other healthcare professionals
  • Educate patients and families on care plans, treatment options, and resources
  • Monitor patient progress and adjust care plans as needed
  • Ensure compliance with healthcare regulations and insurance requirements
  • Advocate for patients to ensure they receive appropriate and timely care
  • Maintain accurate and detailed case management documentation
Qualifications
  • Active Registered Nurse (RN) license in applicable state(s)
  • Minimum 2 years of experience in case management or care coordination
  • Case Management Certification (CCM) preferred but not required
  • Strong organizational, problem-solving, and communication skills
  • Experience with insurance authorizations, discharge planning, and resource allocation
  • Proficiency in electronic health records (EHR) systems
  • Willingness to travel and adapt to different healthcare settings
