AboutRockwell Health Center is a multi-site outpatient healthcare network with clinics in Red Hook, Brooklyn and Astoria, Queens. We are seeking an experienced Licensed Practical Nurse (LPN) to support our primary care operations. You will work closely with our clinical team to coordinate patient care, assist with clinical procedures, and make sure patient needs get timely follow-up.
Key ResponsibilitiesCoordinate durable medical equipment (DME) orders and prior authorizationsCommunicate lab results and follow-up instructions to patients, as directed by the treating providerAdminister vaccines and injections per provider orders and clinic protocolsReinforce patient education on chronic conditions, including diabetes and hypertension, as directed by the clinical teamAssist with medication reconciliation and keep patient medication records accuratePrepare patient letters and support clinical documentation in our Electronic Health Record (EHR), eClinicalWorks (eCW)Track patient follow-ups and flag outstanding needs to providersRoom patients, take vital signs, and step in on medical assistant tasks if neededWork with providers and clinical staff at both sites to keep patient care and clinic flow on trackPerform other nursing duties within the LPN scope of practice, as assigned
RequirementsActive, unrestricted New York State Licensed Practical Nurse (LPN) license and current registrationAt least one year of experience as xzbcwsv an LPNAble to manage a steady patient flow while keeping follow-ups and documentation currentClear, organized communicator with patients, providers, and administrative teamsAvailable five days per week, 9:00 AM to 5:00 PM, on-site at our Red Hook or Astoria clinic
Preferred skillsExperience in primary care, urgent care, or an emergency departmentSpanish proficiencyCurrent Basic Life Support (BLS) certification
