CenterWell Home Health, part of Humana Inc., is seeking a VSP Nurse Practitioner to deliver high-quality in-home care to seniors and adults with chronic conditions. In this role, you will perform comprehensive assessments, diagnose and manage acute and chronic illnesses, prescribe treatments, and develop individualized care plans that help patients recover, manage their conditions, and remain safely at home. Working closely with an interdisciplinary team of nurses, therapists, and social workers, you’ll coordinate care, support smooth transitions, and ensure personalized, patient-centered service. You will educate patients and caregivers on disease management, medications, and safety, while documenting all care in an EMR system.
