Care Manager

JOB DESCRIPTIONInsight Global is seeking a Care Manager (RN) for a top healthcare services client. This individual will lead care coordination efforts for high‑risk patients, ensuring smooth transitions across care settings and appropriate utilization of healthcare resources.

The ideal candidate is clinically strong, patient‑focused, and comfortable working both on-site and telephonically, while partnering closely with providers and multidisciplinary care teams to improve outcomes and the patient experience.
Day‑to‑Day:Ensure continuity of care across inpatient and outpatient settingsLead care management efforts in partnership with PCPs and the care teamIdentify and follow high‑risk patients across acute, SNF, custodial, and ambulatory settingsCoordinate treatment plans and appropriate utilization of resourcesConduct patient and family education related to care plans and transitionsSupport discharge planning, follow‑up appointments, home health, and community resourcesUse disease management protocols and care pathways to optimize outcomesPerform telephonic triage and motivational interviewing to support self‑managementDocument care activities in the EHR and care management systemsCollaborate closely with physicians, hospitalists, and extended care facilitiesTravel locally or provide remote coverage as business needs require
REQUIRED SKILLS AND EXPERIENCEActive Florida RN licenseGraduation from an accredited school of nursingBLS / CPR certification (AHA or Red Cross; or ability to obtain within 90 days)Prior experience in acute clinical care or equivalentExperience in care management, utilization review, or discharge planning xzbcwsv Strong knowledge of HIPAA and patient confidentialityAbility to work collaboratively with providers and multidisciplinary care teamsStrong patient communication, organization, and multitasking skills

Location: Onsite 5 days a week in Dr PhillipsPay: up to $90k
