We are seeking an experienced and detail-oriented Surgery & Pathology Medical Coder responsible for accurately reviewing medical records and assigning appropriate CPT, HCPCS, and ICD-10-CM codes for surgical procedures and pathology services. The role requires a strong understanding of surgical documentation, pathology reports, coding guidelines, modifiers, bundling rules, and payer-specific requirements.
The ideal candidate should have strong analytical skills, excellent knowledge of medical terminology and anatomy, and the ability to interpret complex clinical documentation while maintaining high standards of coding accuracy and compliance.
Key Responsibilities: (Surgery with Pathology Experienced Coders)
Review and analyze operative reports, surgical notes, pathology reports, physician documentation, and other relevant medical records. Assign accurate CPT, HCPCS, and ICD-10-CM codes based on documented services and diagnoses. Perform coding for various surgical procedures, including outpatient and inpatient surgical cases, as applicable. Review pathology and laboratory-related documentation and accurately code pathology services based on the procedures performed and documentation available. Apply appropriate CPT modifiers , including but not limited to 25, 26, 50, 51, 52, 53, 59, 76, 77, and 78, wherever applicable. Identify and apply appropriate global surgery package guidelines and postoperative services coding rules. Ensure accurate coding of surgical procedures, biopsies, excisions, resections, endoscopic procedures, and other applicable specialties. Review pathology specimens and documentation to determine the appropriate level and type of pathology service coding. Identify bundling and unbundling issues and ensure compliance with NCCI edits and payer-specific coding guidelines. Validate medical necessity and ensure diagnosis codes appropriately support the services billed. Identify coding discrepancies, missing documentation, and potential compliance issues and escalate them as required. Maintain coding accuracy and productivity in line with organizational and client-defined quality standards. Perform coding audits and participate in quality improvement initiatives when required.Stay updated on annual changes to CPT, HCPCS, ICD-10-CM, AMA guidelines, CMS regulations, NCCI edits, and payer-specific policies.
Work closely with auditors, QA teams, billing teams, and operations teams to resolve coding-related issues.
Maintain strict confidentiality of patient information and comply with HIPAA and applicable data privacy regulations.
Required Skills and Competencies: Strong knowledge of CPT, HCPCS, and ICD-10-CM coding systems . Good understanding of surgical terminology, anatomy, physiology, and medical terminology. Strong knowledge of surgical coding and pathology coding guidelines. Ability to interpret operative reports and pathology documentation accurately. Knowledge of global surgery rules, bundling concepts, NCCI edits, and modifier application. Strong analytical and decision-making skills. Excellent attention to detail and accuracy. Ability to meet defined productivity and quality targets. Good written and verbal communication skills. Ability to work independently as well as collaboratively within a team.
Qualification Required:
Bachelor's degree in Life Sciences, Nursing, Allied Health, or a related field is preferred.
Medical Coding certification such as CPC, CCS, CCA, or equivalent is preferred.
Relevant experience in surgery and/or pathology coding is required.
Candidates with experience in US healthcare medical coding and RCM processes will be preferred.
Experience Required:
Minimum 3–8years of relevant experience in Surgery and/or Pathology Medical Coding.
Experience working with US healthcare providers, hospitals, physician groups, or RCM organizations is preferred.
Shift : Straddle
