Inpatient Healthcare Social Worker

Intermountain Health seeks a Social Work Care Manager for our inpatient team in Salt Lake City. In this role, you will complete psychosocial assessments, coordinate complex discharges, and connect patients and families with community resources. You will collaborate closely with physicians, nurses, and other disciplines to create safe, patient-centered care plans, provide crisis intervention and brief counseling, and advocate for social and financial needs.
