Provides home-based occupational therapy evaluations and treatments for patients with routine to complex needs. Develops and adjusts plans of care, assesses progress, recommends adaptive equipment and home modifications, documents clinical services, and collaborates with patients, families, physicians, and the care team to support functional abilities and safety.
