Overview As a Care Transitions Coordinator (Clinical Liaison), you will assist patients with the process of navigating post-acute care. Your goal is to create a positive impact on patient outcomes while increasing referral source satisfaction with Enhabit's services. Responsibilities Integrate evidence-based clinical guidelines, preventative guidelines, protocols, and other metrics in the development of transition plans that are patient-centered and promote quality and efficiency in the delivery of post-acute care.
