Nurse Practitioner - NP

The Choice Hospice Nurse Practitioner is a passionate provider and hospice advocate- a proven clinician responsible for helping shepherd the care of the seriously ill. The ideal candidate will have a personality type that is predispositioned to thrive in an unstructured environment and is ok with reacting to patient needs while being a supportive team member with excellent ability to communicate with team members. As a Hospice Nurse Practitioner, you will: Collaborates with the IDG in assuring appropriate evaluation and palliative treatment of terminal disease. Consults with and educates the patient/family regarding disease process, self-care techniques, end-of-life care. Provides training to other staff as needed. Perform Face-to-Face visits at the patient's place of residence. Interacts
