Pride Health is hiring a Physical Therapist (PT) - Home Health to support our client's healthcare facility in Richmond, VA.
Job Details:
Schedule: Full-Time | 8 Hours Per Day | Monday to Friday
Location: Richmond, VA, 23226
Float / Coverage Area: a { text-decoration: none; color: #464feb;}tr th, tr td { border: 1px solid #e6e6e6;}tr th { background-color: #f5f5f5;}Local Travel Required
Job Type: Local contract
Contract Length: 14 Weeks
Pay Range: $60.00 to $66.00/hour*
Mileage Reimbursement Rate (if applicable): $0.50/ mile
*Pay offered is based on experience, expertise, credentialing, and education.
Duties:
Conduct patient evaluations and develop/update individualized Plans of Care (POC).
Provide treatment for neurological, orthopedic, and neuromuscular conditions in the home setting.
Educate patients and caregivers on mobility, transfer techniques, assistive devices, home safety, and therapeutic exercises.
Complete timely clinical documentation, progress notes, and care coordination activities while collaborating with physicians and care teams.
Requirements:
Active State Physical Therapist (PT) License
Current CPR Certification
Valid Driver's License, Reliable Transportation & Insurance
1-2 years of PT experience in acute care, rehabilitation, or related settings
Bachelor's Degree in Physical Therapy from an APTA-approved program
