We are looking for a relationship-driven, self-motivated Community Liaison to grow our agency’s presence in the community and drive new client admissions. This part-time role (approximately [20–25] hours per week) is responsible for developing strong referral partnerships, executing a consistent outreach route, converting inbound leads, and representing our brand with professionalism and empathy. You will play a key role in helping families access the care they need while supporting the continued growth of our agency. This is an ideal fit for someone who is energized by meeting people, works well without daily supervision, and wants to see a direct line between their effort and their results.GROWTH OPPORTUNITY
This position is intentionally structured as part-time so the right person can build the territory the right way. As referral relationships and admissions grow, the role is designed to expand — and once mutually agreed-upon performance and growth benchmarks are consistently met, BrightStar Care intends to transition this position to full-time with expanded compensation and benefits. Specific benchmarks will be established and reviewed with the Branch Owner during onboarding and revisited at regular intervals.KEY RESPONSIBILITIES
• Referral Partner Development o Build and maintain strong relationships with referral sources including hospitals, SNFs, ALFs, hospice programs, physicians, and community organizations. o Conduct regular in-person visits, drop-ins, tours, and networking events (10-20 quality visits per week). Share value-added updates such as availability, success stories, care offerings, and educational materials. o Represent BrightStar Care at chamber meetings, health fairs, CEU events, and community functions. o Host or coordinate in-services, lunch-and-learns, and community events. Lead Generation & Conversion o Generate new business through regular visits, calls, and networking events. o Identify, cultivate, and maintain relationships with referral sources such as hospitals, physician offices, senior living communities, rehabilitation centers, case managers, and social workers. o Respond to all new inquiries within 1–2 hours during scheduled working hours. o Conduct discovery conversations to understand client/family needs. o Guide families through service options and next steps. o Maintain a high lead-to-client conversion rate in alignment with agency goals. o Meet or exceed established sales targets and conversion metrics. • Sales Strategy & Route Management o Develop and manage a weekly outreach route organized by geographic zones and partner tiers. o Promote and cross-sell the full range of BrightStar Care services—including personal care, skilled nursing, and medical staffing. o Build and maintain tiered referral-source lists and track relationships in CRM systems o Track all activities in the CRM, including visits, outcomes, follow-ups, and referral trends. o Review data weekly to identify opportunities, gaps, and priority accounts. • Collaboration & Communication o Work closely with the scheduling, nursing, and operations teams to ensure timely and seamless service onboarding. o Participate in weekly case coordination and pipeline meetings. o Assist with events, community initiatives, and marketing campaigns as needed. o Provide feedback and follow-up updates to referral partners after client admissions. o Work closely with clinical and operations teams to ensure smooth onboarding of new clients and alignment of services. o Support local marketing campaigns and sponsorship opportunities. o Submit weekly sales activity summaries and territory performance reports. SKILLS/ABILITIESKnowledge of referral sources such as SNFs, hospitals, or physician offices. Understanding of consultative or relationship-based selling approaches. Strong communication, presentation, and relationship-building skills. Ability to work independently, manage route, stay organized and manage time effectively. Experience with CRM systems and Microsoft Office. HUBSPOT Familiarity with community networking, chamber events, and partnership-building. Valid driver’s license and reliable transportation.Details
