CenterWell Home Health, part of Humana Inc., is seeking a Physical Therapist Assistant to provide compassionate in-home therapy for seniors and adults with chronic conditions. In this role, you’ll follow treatment plans established by a licensed Physical Therapist, deliver therapeutic exercises, and help patients improve strength, balance, and mobility in the comfort of their homes. You’ll educate patients and caregivers, document progress, and collaborate with an interdisciplinary team that values respect, integrity, and professional growth while supporting independence and quality of life.
