As an RN Care Coordinator in the ED at Kaiser Permanente, you will integrate frontline clinical expertise with innovative digital tools to streamline care for patients in a fast-paced emergency environment. You will assess patients, design and coordinate care plans, and manage safe transitions from the ED to inpatient, outpatient, or community settings. Partnering closely with physicians, social workers, and IT-enabled workflows, you will use EHR and analytics to improve patient flow, reduce readmissions, and enhance outcomes, while contributing to a culture of collaboration and continuous improvement.
