RN Primary Case Manager

The Hospice RN Primary Case Manager provides coordinated end-of-life nursing care in patients’ homes or facilities. This role completes comprehensive assessments, develops and updates individualized plans of care, manages symptoms, and educates patients and caregivers on hospice philosophy and treatment options. The nurse collaborates closely with the hospice interdisciplinary team to coordinate services, support quality of life, and guide families through care transitions and the hospice experience.
