Hospice Physician

Gentiva is seeking a Hospice Physician to provide compassionate, home-based end-of-life care. You will conduct patient assessments, develop and oversee individualized care plans, manage complex pain and symptoms, and collaborate closely with nurses, social workers, chaplains, and therapists. The role includes family counseling, goals-of-care discussions, and ensuring care aligns with patient values. You will support interdisciplinary team meetings, maintain clinical documentation, uphold regulatory and quality standards, and champion Gentiva’s culture of dignity, empathy, and clinical excellence in every patient interaction.
