LPN Nurse Liaison

ChristianaCare is seeking an LPN Nurse Liaison to coordinate smooth transitions of care for patients across our health system. In this role, you will serve as a key point of contact between patients, families, providers, and community services, ensuring timely referrals, accurate clinical information exchange, and safe discharge planning. You will assess patient needs, educate patients and caregivers, support care plans, and advocate for resources that promote optimal outcomes. Working within a collaborative, multidisciplinary team, you’ll contribute to ChristianaCare’s mission of patient-centered, compassionate, and equitable care.
