We are seeking an Occupational  Therapists (OT) to join supportive, patient-focused therapy team in a skilled nursing facility .

Responsibilities

• Evaluate patients and develop individualized treatment plans
• Provide skilled physical therapy services to help patients improve mobility, strength, balance, and independence
• Collaborate with nursing, therapy, physicians, and other members of the interdisciplinary team
• Document patient progress and adjust treatment plans as appropriate
• Provide patient and caregiver education
• Support safe discharge planning and continuity of care

Qualifications

• Current California Occuaptional Therapist license
• Graduate of an accredited Occupational Therapy program
• Strong communication and patient-care skills
• Multiple Sacramento-Area Locations
