The role completes and audits MDS 3.0 assessments, coordinates interdisciplinary care plans, and ensures accurate, timely clinical documentation to support quality outcomes and appropriate reimbursement. Working in a resident-centered, collaborative environment, the MDS Coordinator partners with nursing, rehab, and medical teams to monitor clinical changes, maintain regulatory compliance, educate staff, and promote dignity, safety, and quality of life for residents.
