Bilingual AOT Care Coordinator

Job Type Full-time Description <p> Mental Health Association of Nassau County (MHANC) is a not-for-profit membership organization dedicated to improving mental health in our community through advocacy, education, program development and the delivery of direct services. Our organization is looking for a motivated individual with a calling to help young people with mental illness by becoming a Bilingual AOT Care Coordinator. </p><p> The AOT Care Coordinator functions as a member of an interdisciplinary team to provide care coordination to a caseload of severely mentally ill adults court ordered to participate with a medication and treatment regimen. The AOT Care Coordinator supports the client, engages with community agencies/health care providers and others on the members behalf to ensure access to services needed to increase wellness, self-management and reduce emergency room visits and/or hospitalizations. The AOT Care Coordinator provides support to the team by providing consultation, education, information around psychosocial and/or substance abuse conditions, interventions and resources to maintain focus on outcomes and best practices. </p><p> Essential Duties and Responsibilities </p><ul><li> Conducts initial and ongoing assessments of assigned clients to document strengths, needs, goals and resources. </li><li> Maintains electronic case files and keeps the Supervisor/Director informed and up-to-date on client issues. </li><li> Participates in the development/documentation/review and update of client centered comprehensive, integrated, interdisciplinary care plan in consultation with the Care Manager and other team members to ensure focus on desired outcomes. </li><li> Maintains effective communications with clients, primary care physicians, substance abuse and mental healthcare providers, family, collateral resources and other agency staff on behalf of clients. </li><li> Provides weekly documentation to the County on the progress of AOT clients. </li><li> Maintains documents, records, statistics, and other related reports in an organized, timely and accurate manner. </li><li> Coordinates care planning with other providers of services/resources to ensure goal directed collaborative care, including care transitions. </li><li> Works as a part of a Care Coordination team; attends and participates in team meetings to provide input/feedback around psychosocial conditions to review client status, update plans and goals and review outcomes to further program goals. </li><li> Participates in staff meeting as well as bi-weekly supervision meeting with Supervisor/Director. </li><li> Engages clients to participate in full treatment as listed in AOT order. </li><li> Provides telephonic as well as face to face outreach, engagement and service planning in the field. </li><li> Acts as a linkage to community services including medical, behavioral, residential, substance abuse, entitlement and any other needed services per interdisciplinary care plan. </li><li> Monitors overall service delivery to clients to ensure coordination and continuity; advocates with service providers/resources as needed. </li><li> Provides crisis intervention and follow-up. </li><li> Other duties as assigned. </li></ul> Requirements <p> Education &amp; Experience: </p><ul><li> Masters Degree with One year of Experience with providing direct services to people with Serious Mental Health Illness, Developmental Disabilities, alcoholism or substance abuse </li><li> Masters Degree in: Social Work, Psychology, Education, Rehabilitation, Occupational Therapy, Counseling, Community Mental Health, Sociology, Speech and Hearing, Physical or Recreational therapy. </li><li> Bachelor level candidates, Two (2) years of Experience with providing direct services to people with Serious Mental Health Illness, Developmental Disabilities, alcoholism or substance abuse </li><li> Degree in: Social Work, Psychology, Education, Rehabilitation, Occupational Therapy, Counseling, Community Mental Health, Sociology, Speech and Hearing, Physical or Recreational therapy. </li><li> Credentialed Alcoholism and Substance Abuse Counselor (CASAC) and Two years of experience </li><li> Bachelors Degree or higher in any field with either 3 years of experience or 2 years of experience as a Health Home Care Manager servicing SMI or SED. </li></ul><p> Other Qualifications: </p><ul><li> Bilingual in Spanish and English required </li><li> Experienced in and demonstrated comprehensive understanding and working knowledge of the interdisciplinary planning process and the developmental treatment model. </li><li> Knowledge of Medicaid, Social Security and other entitlements preferred. </li><li> Excellent interpersonal skills required. </li><li> Must have the ability and willingness to regularly travel to various locations in Nassau County, using agency vehicle when transporting consumers. </li><li> Valid NYS Drivers License; must take defensive driving course through agency or external provider </li><li> Working knowledge of computer software (Microsoft Word, Outlook) and electronic health records systems </li><li> Strong verbal and written communication skills required as well as detailed oriented, organized and ability to handle multiple projects at the same time. </li></ul>
