Ascension is seeking a Registered Nurse Patient Navigator to guide patients and families through the healthcare journey. In this role, you’ll coordinate care across clinics and hospital services, provide education on diagnoses and treatments, and promote adherence to care plans. You will collaborate with physicians, multidisciplinary teams, and community resources to remove barriers to care, support transitions between settings, and advocate for patient needs. Ideal candidates are compassionate RNs with strong clinical assessment, care coordination, communication, and patient education skills in a fast-paced, team-based environment.
