Acute Care Transition Registered Nurse

Tufts Medical Center is seeking a Registered Nurse for our Acute Care Transition Team. In this role, you’ll coordinate safe transitions of patients from hospital to home or post-acute settings, emphasizing education, discharge planning, and continuity of care. You will assess patient needs, develop individualized care transition plans, collaborate closely with physicians, social workers, and community providers, and monitor outcomes.
