AVLO Health is seeking experienced Family Nurse Practitioners (FNP/NPs) to provide concierge healthcare in members’ homes and other private settings.We are looking for more than strong clinical skills. AVLO providers must bring sound judgment, professionalism, warmth, discretion, responsiveness, and genuine attention to the member experience.

What You’ll DoProvide primary, preventive, and urgent care during home visitsAssess and manage acute and chronic conditions within your scope of practiceProvide services such as IV therapy, phlebotomy, wound care, suturing, and minor procedures based on demonstrated competenciesRecognize clinical concerns and appropriately escalate when neededEducate members and coordinate follow-up careComplete timely and accurate EMR documentationCommunicate professionally with members, families, and the AVLO care teamProvide a discreet, personalized, high-touch experience from arrival through follow-upTake ownership of concerns and help ensure members feel heard, informed, and cared for
What We’re Looking ForActive, unrestricted New York State Nurse Practitioner and RN licensesNational FNP certificationCurrent BLS certificationRelevant primary care, urgent care, or similar clinical experienceStrong assessment and independent clinical decision-making skillsExcellent communication, documentation, and bedside mannerProfessional presence and discretion when working in private homes and settingsEmotional intelligence, adaptability, and a solution-oriented mindsetAbility to work independently while knowing when clinical escalation is necessaryReliable transportation and ability to travel within assigned service areas
Preferred experience: urgent care, emergency care, primary care, mobile/home-based care, infusion/IV therapy, phlebotomy, wound care, suturing, or minor procedures.
