You will collaborate with behavioral health, pharmacy, OBAT, reproductive health, dental and optometry teams to deliver high-quality care. Responsibilities include managing chronic diseases and acute conditions across the lifespan, providing preventive services, performing reproductive health procedures, and maintaining comprehensive electronic health
