Job DetailsJob Location: Brookhaven Nursing and Rehabilitation Center - Carrollton, TX 75010Job Shift: DayThe PRN Licensed Practical Nurse (LPN) provides direct nursing care to residents in accordance with federal, state, and local regulations, facility policies, and physician orders. The LPN is responsible for promoting resident health, safety, comfort, and well-being while maintaining the highest standards of nursing care. PRN staff work on an as-needed basis and may be scheduled to cover shifts, vacations, call-outs, or increased staffing needs.

Essential Duties and Responsibilities

Resident Care

Administer medications and treatments as prescribed by physicians.

Monitor residents for changes in condition and report significant findings to the physician and supervisor.

Perform nursing assessments within the scope of LPN practice.

Provide wound care and other treatments as ordered.

Assist residents with activities of daily living as needed.

Ensure resident comfort, dignity, and privacy at all times.

Respond promptly to resident needs, emergencies, and call lights.

Documentation

Accurately document nursing care, observations, treatments, and resident responses.

Complete medication administration records (MARs) and treatment administration records (TARs).

Maintain accurate and timely charting in the electronic medical record system.

Report incidents and complete required documentation according to facility policy.

Communication

Communicate effectively with residents, families, physicians, and interdisciplinary team members.

Report resident concerns, changes in condition, and care needs to appropriate personnel.

Participate in shift reports and resident care conferences as required.

Collaborate with Certified Nursing Assistants (CNAs) and other healthcare staff to ensure quality resident care.

Regulatory Compliance

Follow all facility policies and procedures.

Comply with federal, state, and local regulations governing long-term care facilities.

Adhere to infection control, safety, and emergency preparedness protocols.

Maintain resident confidentiality in accordance with HIPAA regulations.

Provide guidance and direction to CNAs and other assigned staff.

Monitor and ensure completion of delegated resident care tasks.

Report performance concerns to nursing leadership.

Qualifications

Current, unrestricted Louisiana Licensed Practical Nurse (LPN) license.

Current CPR certification or ability to obtain within required timeframe.

Previous long-term care experience preferred but not required.

Ability to read, write, speak, and understand English.

Strong clinical, organizational, and communication skills.

Ability to work independently and as part of a healthcare team.
