Position Summary: The MRSS Case Manager is responsible for providing crisis intervention, stabilization services, care coordination, and ongoing support to children, youth, and families in the community. This position provides immediate, mobile, and short-term interventions to promote safety, emotional well-being, and connection to appropriate services and resources. The Case Manager works collaboratively with families and community providers to ensure services are delivered according to the MRSS model and in alignment with Unison Health’s mission and standards. Key Responsibilities & Role Highlights:
  • Provides transportation to clients and families as needed to support service delivery and access to appropriate resources.
  • Documents all services provided and maintains accurate client records using the electronic health record in accordance with agency policies and procedures.
  • Participates in team meetings, supervision, in-service training, and other required conferences in accordance with established policies and applicable regulations.
  • Maintains client confidentiality and ensures all releases of information are handled in accordance with federal guidelines and agency policies and procedures.
Education & Experience Requirements:
  • A bachelor’s degree in related field Preferred or 3 years or more past experience. Experience with youth preferred.
  • Must demonstrate knowledge of mental illness including the SED population, crisis management and the ability to establish and maintain therapeutic relationships with clients.
  • Must demonstrate the ability to define problems, analyze data, establish facts, draw valid conclusions, and make recommendations.
  • Must demonstrate positive attitude, commitment to provide superior services and enthusiastic support for the mission of the agency.
  • Must establish and maintain effective working relationships.
  • Must demonstrate the ability to represent the agency in a positive manner when in the community.
  • Must be able to work with ill, disabled, emotionally upset and at times hostile individuals.
