Clinical Care Program Coordinator, Diabetes (Intern)
Sugar360 · India
Job Description
Company Description
Sugar360 is a unified diabetes-care ecosystem focused on continuous, personalized, and outcome-driven care. The organization combines clinical expertise, lifestyle guidance, and technology to support individuals beyond clinic visits, enabling better monitoring and stronger engagement. By bridging the gap between traditional episodic care and ongoing diabetes management, Sugar360 creates a connected system that empowers patients and supports healthcare professionals. The team is driven by a vision to help every person with diabetes live their healthiest life and a mission to improve and maintain the quality of every diabetic life it touches.
About the role
We're looking for someone to run our Care Delivery System on real patients, from day one. The CDS is the protocol that governs everything a patient experiences across their 90-day program, the coach calls, the nudges, the check-ins, the follow-ups. You'll own it end to end across our first two live cohorts in Siliguri, starting with 15 patients and moving to a second group of 15 to 20.
This is not a shadow-and-observe internship. You'll be the person the coaches report to, the person who knows every patient's status, and the person who decides what gets escalated to a doctor. You'll also be the one who finds out where the protocol breaks in practice and documents what needs fixing before we take it to franchise clinics outside Siliguri.
What you'll do
Own the CDS protocol end to end, making sure every enrolled patient moves through the same sequence of touchpoints without anything slipping
Direct the health coaches day to day, including who they call, when, and what each call is meant to achieve
Make sure every new patient completes intake properly before they enter the 90-day program
Track patient status closely: appointments attended, nudges sent, glucose and diet logs, and early signs someone is about to drop off
Chase down and re-engage anyone who goes quiet, since retention is the whole point of the model
Decide what you can handle within protocol and what needs to go to a doctor or the founder
Capture the data we need to judge each cohort honestly, including adherence, dropout, and clinical markers
Keep both cohorts running the same way, so results reflect the model and not who happened to be executing it
Write down every place the protocol doesn't work as intended, and keep the playbook updated for handover to future clinic locations
What we're looking for
A background in Nutrition and Dietetics, or a qualification in nursing or allied health with real patient-facing experience
MSc Clinical Research or MPH students are welcome, including as a dissertation or practicum placement. We'll report to your university supervisor and sign your institution's placement agreement
PhD scholars in a clinical, nutrition, or public health field are also welcome to apply
Strong people skills, because this role is about patients and coaches, not spreadsheets in a back office
Comfortable holding health coaches accountable to protocol, even ones more senior than you
Comfortable with things being unfinished, since we'll be fixing the protocol while running it
Careful with detail, because adherence and outcomes data is worthless if the record-keeping is sloppy
Working knowledge of Excel or Google Sheets. CRM experience is a plus, not a requirement
