Category Pharmacy - Clinical Pharmacist Job Type Direct Hire A direct hire opportunity is available for a Clinical/Infusion Pharmacist in the Columbus, OH area, offering a Monday through Friday schedule from 8:00 AM to 4:30 PM with participation in on-call rotation. This role focuses on delivering specialized pharmaceutical care in a home infusion setting. Responsibilities include preparing sterile specialty pharmaceuticals and intravenous solutions in compliance with regulatory standards, clinical monitoring of medication therapies including pharmacokinetic dosing and Total Parenteral Nutrition (TPN), and providing thorough patient counseling on the use and management of infusion therapies. The pharmacist will educate patients and caregivers on infusion pump operation, sterile techniques, and catheter maintenance to ensure safe and effective home treatments. Collaboration is key, as the pharmacist will work closely with physicians, home health nurses, and various internal teams such as intake, clinical services, and pharmacy operations to coordinate care and reduce hospital readmissions. Maintaining compliance with a range of regulatory standards from State Board of Pharmacy requirements to HIPAA and OSHA is an essential part of this role. Additionally, managing pharmacy inventory and overseeing the packing and timely delivery of medications to patients' homes will be required. Desired Qualifications: Minimum of 2 years' experience in infusion specialty pharmacy IVIg experience preferred Strong knowledge of specialty therapy payer requirements, reimbursement processes, and contract obligations Current and valid Ohio pharmacist license in good standing Experience with CareTend system is advantageous This direct hire position invites pharmacists with expertise in clinical and home infusion services to contribute to a multidisciplinary care model.
