You will evaluate functional abilities, develop and implement individualized treatment plans, and help residents improve independence in daily living and mobility. Collaborate closely with nursing, rehab, and care teams to ensure safe, person-centered interventions. Provide caregiver training, document progress, and contribute to discharge and care planning.
