Kaiser Permanente seeks a Licensed Social Worker to deliver compassionate, patient-centered care within our Healthcare & Medical Services department. In this role, you will conduct psychosocial assessments, develop care and discharge plans, provide counseling and crisis intervention, and coordinate services across an interdisciplinary care team. You will connect patients and families with community resources, support care transitions, and contribute to quality improvement.
