Home Health Speech Language Pathologist

CenterWell Home Health is seeking a PRN Home Health Speech Language Pathologist to provide skilled speech therapy to adult and senior patients in their homes. In this role, you will assess communication and swallowing disorders, develop and implement individualized care plans, and educate patients and caregivers on strategies to improve function and safety. You’ll collaborate with an interdisciplinary team to coordinate care, document visits accurately, and ensure regulatory compliance.
