Responsible for coordinating with others to establish, implement, and evaluate the palliative care program’s operational and clinical services in compliance with organizational policies, procedures, applicable regulations, and quality standards to contribute to the organization’s mission of providing patients with access to quality palliative care.
What you will do:
- Coordinate and oversee all direct and indirect patient services provided by clinical personnel.
- Plans, establishes, and implements program elements and evaluates quality of care provided to patients.
- Tracks and reports on identified quality metrics and performance improvement activities.
- Analyzes operational indicators to identify census changes and increases or decreases in volume that could impact staffing levels, revenue, and/or expenses.
- Develops methods and procedures to ensure program services are delivered in accordance with policy and organization mission and goals and to meet patient and community needs.
- Reports to the Vice President of Patient Access and coordinates policy and operational activities with the President of the palliative care group practice, Hudson Valley Medical Health Choices.
- Develops and implements opportunities for additional or improved services.
- Reviews and audits EMR documentation for quality and compliance.
- Provides guidance and education to the palliative care team to assist them in continually improving all aspects of services provided.
- Visits referring physicians, group practices, ACOs, and insurers to assist in identifying patients who would benefit from palliative care services.
- As necessary, evaluate patients for admission to palliative care services.
- Coordinates MD and NP clinical activities to ensure services are properly and completely billed to patients and/or third-party payors.
- Works with Finance Billers to ensure all services are properly accounted for and billed.
- Oversees care coordination activities provided by Palliative Care physicians, Nurse Practitioners, Palliative Care RNs, Social Workers, and Referral Center Coordinator

Work Environment:
The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.
- Works in the office and other assigned settings
- May be required to safely lift up to 50 pounds
- May be required to speak/present to groups of people
- May be required to visit referring physicians and patients
Preferred Education & Experience:
- Graduate of an accredited nursing school; BSN preferred
- Current unrestricted NYS Registered Nursing License
- Minimum of 3 years management experience in a health care setting, preferably in hospice operations.
- Care management, case management, or triage experience preferred
- CHPN Certification preferred
- Valid NY Driver’s License
Hours: Monday-Friday (8:30am-4:30pm); Supervisor on call rotation (2-3 times per year)
