Inpatient RN Care Manager I

Mercy is seeking a dedicated RN Care Manager to support inpatient care coordination and safe transitions across the continuum. In this role, you will perform comprehensive nursing assessments, develop and implement patient-centered care plans, and collaborate with physicians, therapists, and social workers to optimize clinical and financial outcomes. You will lead discharge planning, educate patients and families, ensure appropriate post-acute services, and document thoroughly in the EHR.
