JOB DETAILS POSITION SUMMARY - RN Care Manager - Hospice POSITION DUTIES - Ability to manage patient care and provide services that require substantial, specialized bedside nursing skills for symptom management, crisis intervention and family education. In addition to excellent written/verbal communication, problem solving, and decision-making abilities, the candidate should demonstrate experience with developing and maintaining rapport with patients and families, and the ability to work well with an interdisciplinary team. MINIMUM REQUIRED QUALIFICATIONS - VA/compact RN license BLS Valid driver's license Good driving record and reliable transportation 1+ year experience in hospice PREFERRED QUALIFICATIONS - LENGTH OF ASSIGNMENT - 13 weeks SHIFT / HOURS PER WEEK - Flexible M-F 8:30-5. Will Consider 4/10's.
