Key Responsibilities:Provide primary care for routine and acute conditions.Conduct annual wellness visits and health risk assessments.Coordinate patient care with specialists and healthcare teams.Educate patients on diagnoses, treatments, and care plans.Support population health initiatives and patient outreach.
Required Qualifications:MSN, DNP, NP Certificate qualification.National certification in an eligible NP specialty or Physician Assistant certification.Active, non-probationary state NP license (for NPs).Active DEA license.Authorization to work in the United States.Fluency in Spanish is required. xzbcwsv
Preferred: Experience in primary care, internal medicine, geriatrics, or family medicine, with knowledge of ICD-10 coding, HCC documentation, and health risk assessments.
