• Responsibilities include :
  

  • Routine medical evaluations and chronic disease management.
    

  • Annual wellness and preventive care assessments.
    

  • Management of dementia, frailty, and other complex chronic conditions is common in the long-term care population.
    

  • Oversight of palliative and comfort-focused care plans when appropriate.
