LifeBridge Health, Inc. seeks a Case Manager RN to coordinate safe, efficient patient care across the continuum. This role conducts comprehensive clinical assessments, develops and updates individualized care plans, and leads discharge planning to support optimal transitions. The RN collaborates with physicians, social workers, payers, and community resources to ensure appropriate utilization, remove barriers, and support quality, cost-effective outcomes. The Case Manager educates patients and families, documents in the EHR, tracks outcomes, and supports a compassionate, patient-centered, and community-focused care environment.
