Category Pharmacy - Clinical Pharmacist Job Type Direct Hire This direct hire opportunity seeks a Clinical/Infusion Pharmacist dedicated to preparing, dispensing, and monitoring complex intravenous (IV) and subcutaneous medications in a home infusion setting. Located in Westerville, OH, this role supports patients by acting as a key clinical resource within a multidisciplinary team, ensuring safe and effective therapy management. Schedule/Hours: Monday to Friday, 8:00 AM - 4:30 PM Participate in on-call rotation Key Responsibilities: Prepare specialty pharmaceuticals and sterile IV solutions in compliance with state and federal regulations. Assess lab values, perform pharmacokinetic dosing calculations (e.g., vancomycin), and monitor Total Parenteral Nutrition (TPN) therapy. Provide patient counseling and education on specialty medications and infusion techniques. Train patients and caregivers on using infusion pumps, sterile methods, and catheter care at home. Collaborate with physicians, nurses, and pharmacy teams to coordinate care and prevent hospital readmissions. Maintain compliance with regulatory standards, including HIPAA, DEA, Medicare, ACHC, URAC, OSHA, and State Board of Pharmacy guidelines. Manage inventory, including refrigerated products, and ensure timely, safe delivery of medications to patients. Qualifications and Experience: Minimum of 2 years of infusion specialty pharmacy experience. IVIg experience is a plus. Knowledge of specialty therapy payer requirements, reimbursement processes, and contract obligations. Current and active Ohio pharmacist license in good standing.
