Case Manager

WCS Healthcare is seeking a Case Manager to provide temporary coverage for a residential services program. This is an excellent opportunity to support vulnerable populations and ensure continuity of critical client services related to housing, case management, and community support.

Responsibilities

• Conduct intake assessments and orientation for new clients within 24 hours of admission.
• Complete psychosocial, housing, vocational, and educational assessments.
• Provide comprehensive case management services to an assigned caseload.
• Develop and maintain individualized service plans and monitor client progress.
• Assist clients with housing applications, interviews, and placement processes.
• Facilitate individual and group counseling, workshops, psychoeducational groups, and recreational activities.
• Conduct room inspections and assess activities of daily living, hygiene, and overall well-being.
• Coordinate referrals to medical, mental health, housing, and supportive services.
• Monitor medication compliance in collaboration with nursing staff.
• Provide crisis intervention and respond appropriately to emergency situations.
• Assist with discharge planning and housing transitions.
• Maintain accurate, timely, and compliant documentation.
• Attend staff meetings, trainings, supervision, and case conferences as required.

Qualifications

Required

• High School Diploma or GED.
• Ability to work effectively with a culturally diverse population.
• Strong communication, organizational, and interpersonal skills.

Preferred

• Bachelor's degree in Human Services, Social Work, Psychology, or a related field.
• One to two years of experience working with homeless or underserved populations.
• Experience in residential services, case management, housing programs, community outreach, or social services.

• Monday, Tuesday, Thursday, Friday: 8:00 AM - 4:00 PM
• Wednesday: 2:00 PM - 10:00 PM
