Job Details: Schedule: Monday-Friday | 8:00 AM-4:30 PM | 36 Guaranteed Hours/Week | 1 Week and Weekend of On-Call Location: Madison, OH; Coverage areas include Lake, Geauga, and Ashtabula Counties Mileage reimbursed: Yes Job Type: Contract- 13 Weeks Pay Range: $52-$54 *Pay offered is based on experience, expertise, credentialing, and education. Duties: Manage and coordinate patient care as an RN Case Manager. Assess patient needs and develop individualized plans of care. Coordinate services and communicate with physicians and interdisciplinary healthcare teams. Requirements: License(s): Active RN license.
