The Addiction Counselor will provide counseling and support to individuals struggling with addiction, helping them navigate the path to recovery.

Prison Discharge Planner / Corrections  Social Worker / Corrections  Counselor / Drug Recovery and Resettlement

Looking for a Social Worker, Human Services, Criminal Justice or Mental Health Professional with addiction experience to work directly with prisoners that have a history of drug abuse, assist with discharge planning, MAT therapy and appointments, coordinating community resources are in place for reentrants on the date of their release.

TRAINING:

• On-site training provided during shift times on location

Hours:  8a – 4pm (30 min lunch), Monday – Friday

Location : Somerset, PA

Pay:  $

Job Duties:

• Implement and provide social work, counseling, and case management services to individuals to enhance their social functioning and to help them attain a more satisfactory social, economic, or physical adjustment.
• Assist with referrals in the community, including setting up and scheduling appointments for medication and treatment.
• Ensure needs, benefits, and services are obtained in accordance with assigned individuals’ treatment plans.
• Track and update spreadsheet/database on everyone released from the facility
• Meet with reentrants with positive urine drug screens to assess and refer to treatment.
• Complete assessments as required (Initial, 6 months, discharge).
• Communicate all treatment planning to Center Staff, SCI Staff, and/or Parole staff.

Requirements:

• Bachelor’s in Social Worker or Human Service or Similar field required.
• Knowledge and experience in Case Management, Discharge Planning and Community Resources.
• Knowledge of counseling techniques and interventions used to recognize or improve psychosocial, economic, and physiological situations of individuals and families.
• Knowledge of commonly occurring physical and mental illnesses and their social, economic, and psychological implications.
