Transition Social Worker (18-25) Hybrid

Peterborough City Council is seeking an experienced Social Worker to join the 0-25 Transition Team in Peterborough. You will support young people moving from Children's Services into adulthood, conducting Care Act assessments and guiding families through the transition process.The role requires experience in Transition or Adult Social Care, knowledge of the Care Act 2014, and the ability to travel as needed.
