Physical Therapist (PT) – Skilled Nursing

We are seeking Physical Therapists (PTs) to join supportive, patient-focused therapy teams at skilled nursing facilities throughout the greater Sacramento area.

Multiple positions are currently available in Sacramento, Citrus Heights, and Carmichael. Full-time, part-time, and PRN opportunities may be available depending on location.

Responsibilities

• Evaluate patients and develop individualized treatment plans
• Provide skilled physical therapy services to help patients improve mobility, strength, balance, and independence
• Collaborate with nursing, therapy, physicians, and other members of the interdisciplinary team
• Document patient progress and adjust treatment plans as appropriate
• Provide patient and caregiver education
• Support safe discharge planning and continuity of care

Qualifications

• Current California Physical Therapist license
• Graduate of an accredited Physical Therapy program
• Strong communication and patient-care skills
• Multiple Sacramento-Area Locations
