RN Case Manager

Good Samaritan Health Center in Atlanta seeks an RN Case Manager to coordinate holistic, patient-centered care for underserved individuals and families. In this mission-driven, faith-based setting, you will complete comprehensive clinical and psychosocial assessments, develop and manage care plans, and coordinate services across primary care, dental, behavioral health, and community resources. The RN Case Manager promotes health equity through education, advocacy, chronic disease management, and effective care transitions while collaborating with an interdisciplinary team focused on whole-person wellbeing.
