Registered Nurse RN Hospice After Hours

Transitions Care is seeking a compassionate Registered Nurse for Hospice After Hours to provide expert clinical care, triage, and support to patients and families during evenings, nights, and weekends. In this role, you will perform symptom assessment and management, respond to urgent needs, make home or facility visits, and coordinate with the interdisciplinary team to ensure continuity of care. You’ll educate and support caregivers, document thoroughly, and uphold our mission of dignity, respect, and comfort at end of life while working autonomously within a collaborative, values-driven organization.
