Community Care Physicians seeks a Family Medicine NP/PA to join our Slingerlands practice. In this outpatient role, you will provide comprehensive primary care across the lifespan, including acute visits, chronic disease management, preventive care, and wellness counseling. You will work closely with a collaborative team of physicians, advanced practice providers, nurses, and support staff in a patient-centered, physician-owned group. Responsibilities include conducting exams, ordering and interpreting tests, managing treatment plans, documenting in the EHR, coordinating care with specialists, and contributing to quality improvement initiatives.
